| No student who is interested in playing a sport (including trying out for that sport) is allowed to participate at all without first being paperwork ready. You can obtain physical paperwork from the athletic training room, front office, or athletic directors office. All students who want to participate in a sport must have a current physical on file with the athletic trainer. ALL PHYSICAL PAPERWORK MUST COME DIRECTLY TO THE ATHLETIC TRAINER. Please do not place this paperwork in our mailbox or give the information to a coach. If you are a ninth grader, new to our school, or have never played a sport at WSHS before, you must also provide a copy of your birth certificate. We must have your insurance information on file. If you do not have personal insurance, you must purchase school insurance and provide proof of that prior to trying out for a sport. Sports physicals are offered for a fee of $20.00 at the end of every July. This is the perfect time to be sure you are cleared to participate in all sports for the school year. All physicals are only valid for 365 days. If your health status changes during this time, it is extremely important that you contact an athletic trainer. We need to know what allergies you have, if you have a history of head injury, or if you have any history of a medical condition. Please make sure that all paperwork is filled out in its entirety. Please refer to the green checklist stapled to the front of the paperwork if you have questions or do not hesitate to call Sue Ellis at 407-320-8749. |
